  • 23-24 AIA Physical Form
    Packets must be submitted to your myBrophy account by 6/1/23. Students will not be able to participate in team activities until the physical is verified in the system.

    Important Notes to Students
    1. Student’s signature is needed on page 5 of the packet.
    2. ALL signatures on pages 5 & 6 of the packet must be present.
    3. Make sure the student’s name and DOB are filled out (tops of
    pages).
    4. Page 4 answers must be clearly circled.
    5. All pages should be viewable from download prior to
    submission.
    6. We are not granting extensions due to insurance coverage.
    7. On page 3 of the Instructions packet there is a list of low-cost
    providers that they can visit to get their physical completed.
    8. As soon as the 6-page packet is complete submit it to myBrophy.
    9. Students review your packet to make sure EVERYTHING IS
    COMPLETE PRIOR TO SUBMITTING IT. If the packet is
    missing even one piece it cannot be accepted in the system.
    10. If they need assistance (or their submission window in
    myBrophy re-opened) please have them contact the Physicals
