Instructions for Requesting Transcripts for Current Brophy Students

• An unofficial transcript signed by both the student and his counselor must be on file with Mrs. Nelson before a request form is issued.
   
• Parents may not request transcripts for their sons.
   
• No request may be made by phone or via email.
   
• Once the student makes his requests on the form, he must pay $2.00 per school.
   
• We no longer include SAT and ACT scores on our transcripts.  The student must request their scores directly from college board.  For further instructions regarding test scores, students should see their counselor.
   
• The official transcripts are mailed directly from the Brophy.  NO STUDENT MAY HAND CARRY AN OFFICIAL TRANSCRIPT WITHOUT PERMISSION FROM HIS COUNSELOR.
   
• The date the transcript was sent to the requested school will be available through your counselor or Mrs. Novak.
   
• No transcripts will be generated after December 5th.  Requests will be honored when we return from Christmas Break
